CMU’s Intellectual and Developmental Disability Supports Coordination Services are offered to any child or adult who meets the following criteria:

1. A resident of Dauphin County, Pennsylvania
2. Age 3 or older
3. Consent to participate in services by the following means:
   1. Parent/guardian consent for a minor child
   2. Consent by the adult seeking to receive services
   3. Consent by the legal guardian for an adult with a court ordered guardianship
4. A diagnosis of intellectual disability as defined by PA Office of Development Programs (ODP) policy. For more information from ODP, click here.

IDD Intakes are the first step in the Supports Coordination process. These meetings can be scheduled in one of our offices, or at your home. Accommodations such as interpreters are available if needed. When scheduling an Intake, you will be given a list of documents to have available. During the Intake, a friendly and knowledgeable Service Access Coordinator will gather information to determine eligibility in ODP programs.

A listing of documents to have available during the Intake includes:

• All insurance cards
• Educational records
• Psychological reports
• Current physical report from a medical doctor
• Birth Certificate
• Social Security Card
• Drivers License or PA non-drivers photo ID (if applicable)
• Immigration papers (if applicable)
• Financial Information – SSI, income, etc.
• Other information that you have and feel will help the process of determining “eligibility”
• If applicable, guardianship papers, power of attorney papers, and a copy of a living will

If you are deemed eligible for services, another meeting will be arranged to continue the process. If you are deemed to be not eligible for services, our staff will still try to help you connect with resources that may help you.
